Program Overview
One on One appointments with a Registered Respiratory Therapist for high risk COPD patients (Gold level C & D) aiming to improve patient quality of life, family experience, decrease hospitalization and emergency room use, provide medication review and training on proper use as well as coordination of services for patients.
